Submit your Team Request with  Payment to the Golf Committee via our PO Box below.

Members must pay $125/per person and Non-members must pay $220/per person. Anyone who is NOT EMPLOYED by a CGCA member company will be
required to pay Non-member pricing.

 

Your team request will be taken on a first-come, first-served basis, and ONLY after we have received you check for tournament fees. This event often fills to capacity, please submit your team request with payment as soon as possible.

All team requests and payments must be
received No Later Than July 28th!
